Orange
The Transition Age Youth program provides services to youth 16-22 years of age, utilizing the Resiliency Model to meet clients “where they are at.” Providing therapeutic services and support in the youth’s environment helps adolescents and young adults succeed. TAY supports the adolescent or young adult in developing and maintaining caring relationships by being held to high expectations, and giving them opportunities to participate and contribute to their community.

Chittenden
YIT programming provides clinical case management and care coordination services to youth with emotional/behavioral challenges , 16-22 under the same footprint as the JOBS Program. It also provides a young adult navigator through the Spectrum Multi-Youth program and maintains an online resource guide for young adults, Impact VT.
